Personally Identifiable Information Collection

Altient collects information from potential members.  This information is important for us to determine how to tailor our services for your specific needs and how to best contact you for next steps.  As a healthcare provider, or representative thereof, you will be asked to provide your specialty, type of provider, and zip code.  To further check for service availability, sign up as a member, and set up your account, we ask you for information such as facility name, facility type, primary contact name and title, facility address, suite, city, state, zip, facility telephone, facility fax, primary contact telephone, primary contact fax, primary email, secondary email, building management contact name and telephone, a list of all healthcare provider names, address, phone, fax, email, specialty, group affiliations, Medical Education number, and profile information.  We also ask you how you heard about us and if referred, by whom.  We also ask you if you would like to refer a colleague to us and to provide us with contact information for them so we may contact them.  When visiting the website, we automatically collect your Internet Protocol (IP) address, what page you requested, and the time, of which all are not linked to personally identifiable information.

 

Third-Party Information Collection

Absolutely no third-party directly collects personally identifiable information through our Web site. 

 

Altient Use of Collected Information

We store information from you to help us better tailor our services for you as well as qualify your location for broadband Internet access.  The contact information is used to notify you of the progress of your request.  Information is also used by us for internal statistical analysis, internal promotions, and our goals for the commitment of excellence.  Some of the additional information collected on the sign up form is used by our business partners to direct pertinent marketing promotions to our customers, or by our clients to identify their specific market.  This information is shared in aggregate form only, unless you have opted-in for specific information requests.

We periodically collect information for surveys or contests, such as contact information and general demographics.  The information retrieved in the surveys are used to provide better customer service (e.g. satisfy a customer’s needs, enhance a current customer’s experience, provide opportunity for more visible roles, and enhance product development).  Personally Identifiable information is not shared with third parties unless we give prior notice and choice.  If we use an intermediary to conduct these surveys or contests, they may not use your personally identifiable information for any secondary purposes.

 

If you provide us with a referral, we use the contact information you have provided to notify them about our service.  If they choose to not proceed with our services and express that they would like to be removed from our database, we do so at their request.

 

Security is of the highest priority at Altient.  Much of our service is built around effective security because we understand how crucial it is to protect yours and your affiliates’ privacy.  We have many layers of security to protect your information on and offline.  When entering any personally identifiable information online, we use what is called a secure socket layer that protects your information from being exposed to anyone but Altient.  Your personally identifiable information is also stored securely in the office.  Only the security-access employees have access to the information, such as, but not limited to an IT manager (who maintains the data), an accounting staff individual (to process billing information), and a customer service representative (to assist with your account).  The servers that store the information are in a secure location, only accessible by high-level security employees.  If you have any questions about the security at our Web site, please send an email to .  For those of you that currently have our services in your office, we provide you with a firewall.  In addition to the firewall, the wireless network has 128-bit encryption.  Your personally identifiable information is handled the same as above in reference to storage in the office.  Altient cannot guarantee this level of security of personally identifiable information unless the customer is solely on our network and not also networked into another LAN.

 

Choice/Opt-out
